Atresia of the hymen - causes, symptoms, treatment

When is surgical defloration recommended?

Surgical defloration is done in the following cases:

  • Dense tissue of the hymen, which is very painful to tear naturally
  • The elasticity of the hymen, in which it does not break, but causes discomfort during sexual intercourse
  • Fusion of the hymen (occurs most often as a consequence of inflammation in the vaginal cavity)
  • The presence of “first night syndrome” to remove the psychological barrier before sexual intercourse

Diagnostics

The diagnosis of hymenal atresia is made after a gynecological examination. The gynecologist notes such signs of pathology as:

  • Swelling of the hymen;
  • Formation of a tumor between the outer labia the size of an egg;
  • When palpating the ovaries, uterus and lower abdomen, the patient complains of severe pain;
  • Complaints about lack of menstruation;
  • Pathological absence of the vaginal opening;
  • Blueness of the hymen, which indicates the beginning of menstruation and the accumulation of blood and mucus in the uterus.

How is surgical defloration done?

Surgical defloration is performed using surgical instruments, excising excess hymen tissue. In our clinic, according to indications, high-tech surgical equipment can also be used: a radio knife or a surgical laser. In laser defloration surgery, the hymen is corrected using a laser beam. The laser beam cuts the tissue while simultaneously coagulating the blood vessels, which reduces bleeding. During laser surgery, there are no scars or cosmetic sutures left at the treatment sites. Laser surgery is the most gentle method of surgical defloration.

Loss of virginity

Losing virginity is an alarming and unexpected moment in the lives of most young people. The average age of loss of virginity among young people is estimated at 18 years.

A number of social and biological changes that have occurred in recent decades have accelerated the age of sexual initiation. Early age of sexual activity is considered a risk factor for sexually transmitted diseases, emotional stress and unplanned pregnancy. Numerous studies show that virginity loss occurs later in girls.

The loss of virginity is associated with the first sexual intercourse. Love, curiosity or sexual desire become the motives for the loss of virginity, which usually occurs in the family home in the absence of parents and in the lap of nature. Almost half of the cases are spontaneous.

Statistically, virginity loss occurs around the age of 18, but up to 1/3 of sexual initiation occurs before the age of 18. Loss of virginity in men occurs slightly earlier than in women.

Psychosocial consequences of early loss of virginity

Early sexual initiation is associated with various psychosocial consequences. The reasons for this situation are manifested both in the accelerated development of youth and in changes in sexual morality, especially in the lowering of the age of sexual initiation. Premature motherhood is disrupted by the model of life generally accepted in society, in which the following occurs: getting an education, getting a job, professional and financial independence, and only later starting a family. Early loss of virginity often contributes to risky sexual encounters and unrealistic expectations for another sexual partner. In addition, there are medical, endocrine, infectious or obstetric effects of premature intercourse. Teens who initiate sexual intercourse prematurely are less likely to use any contraception in adulthood and have less stable sexual relationships in the future.

Late loss of virginity

The age of sexual initiation is significantly related to educational level. The higher it is, the more likely it is that the loss of virginity will be delayed. This is especially true for girls, as their positive attitude towards school and desire for future exploration raise the age of initiation. Other studies have reported that parental support appears to reduce the likelihood of becoming sexually aroused too early. Delay in sexual activity may be the result of some personal problems in the intimate sphere. On the other hand, it is the result of religion and preserving virtue for the future spouse. Then loss of virginity often occurs after 30 years of age due to the age of marriage. The tradition of maintaining virginity before weddings is observed especially among Catholics and Muslims.

Researchers at the University of Texas reported that the later they lose their virginity, the better they will cope with it later in life. The relationships they create are permanent and very satisfying. They are less likely to have partnership crises. In their opinion, the optimal age for sexual initiation is 20 years. Then the loss of virginity is not accompanied by awkwardness, ignorance of physiology and fear.

According to the research team led by Sandfort, late loss of virginity interferes with normal emotional and cognitive development and negatively affects the development of interpersonal skills. As a result, people who are late in sexual initiation have worse moods and less satisfying lives. According to researchers, late loss of virginity applies to people over 22 years of age.

Opinions are divided on the age at which it is best to lose virginity. In this case, it is important to follow the rule that everyone matures for sex in their own time.

Losing your virginity - what is it like?

For most young people, having sexual intercourse for the first time is a very important life event, which is usually accompanied by worry, fear and worry. Consequently, most of the first sexual intercourse occurs with a close person who can be trusted. Other dedication is associated primarily with romantic experience, pleasure and passion.

Frequent questions that worry young people: what is the loss of virginity? And does it hurt after that? There are many myths in this topic. Most of them concern the interruption of the hymen, that is, the thin and flexible fold of mucous membrane that limits the entrance to the vagina. Loss of virginity is sometimes accompanied by bleeding, an early feeling of tension in the mucous membrane and its rupture. Sometimes there is pain, the intensity of which varies between women. The unpleasant physiological effects of losing virginity will be mitigated by the right atmosphere, relationship with a partner, relaxation and foreplay, which will loosen the body and make the membrane elastic. The best sexual position for losing virginity is the so-called classic missionary position. In it, the vagina is tense, which contributes to the interruption of the hymen.

Benefits of laser defloration surgery

Operation using a laser, unlike surgery, is a completely non-contact method because tissue is removed with a beam of light

  • The laser simultaneously coagulates blood vessels, which reduces bleeding
  • The laser beam disinfects when exposed to tissue, which eliminates the possibility of infection during surgery
  • The laser can perform very precise manipulations and make even minimal tissue incisions
  • When the laser beam is applied to the tissue in a targeted manner, the likelihood of scarring is eliminated
  • The recovery period after laser defloration surgery is very short. Full recovery, including the ability to live an intimate life, occurs after 5 - 7 days after surgery
  • There are no side effects with laser hymenoplasty

How does a woman feel with hematocolpos?

The accumulated blood puts pressure on the vaginal walls, uterus and neighboring organs, causing pain, urination problems and stool retention. The patient experiences a condition called false amenorrhea or cryptomenorrhea, when menstruation occurs, but its external signs are absent.

Blood accumulates in the vagina gradually, so at first pain occurs at intervals corresponding to the menstrual cycle. Later, the accumulated bloody discharge flows into the uterus, and a hematometra occurs (accumulation of blood in the uterine cavity), so the stomach hurts even during the intermenstrual period.

From the uterus, blood flows through the tubes into the abdominal cavity: sharp abdominal pain, nausea and vomiting appear. The condition threatens life and health and therefore requires immediate treatment. When an infection enters the menstrual blood, pus accumulates inside the genital organs. Infection leads to septic or painful shock.

Stomach ache

The disease can be suspected if there are periodic cramping pains and a palpable round formation above the pubis, reaching the size of a tennis ball. When the hymen is fused, upon examination, a dense hymen without an opening is visible, protruding between the labia. Sometimes it is cyanotic due to translucent accumulation of blood.

Hematocolpos with vaginal obstruction in women is easily determined - insertion of the instrument causes a feeling of obstruction and sharp pain. In girls, accumulation of blood caused by vaginal obstruction is detected when examined through the rectum. The diagnosis is confirmed by pelvic ultrasound.

Surgical defloration: contraindications and the operation itself

Surgical defloration does not require special preparation. The main contraindications against surgery are the presence of diseases such as syphilis, hepatitis and HIV, as well as the presence of active inflammation in the vaginal area.

The operation is performed at the beginning or middle of the menstrual cycle, at least 7 days before the start of menstruation.

The operation itself takes about an hour of pure time, including preparation and anesthesia. The patient remains in the room for 2 hours after the operation to let the anesthesia wear off, after which he can leave the clinic on his own. The operation is performed on an outpatient basis and anonymously.

The reasons for its occurrence

With the congenital version of this pathology, the culprit of the situation is a certain failure during the development of the fetus. It is necessary to understand that the formation of the hymen occurs in the 19th week of pregnancy. The paramesonephric canal takes part in its formation, with the subsequent formation of the necessary holes in the hymenal septum. In case of any failure of this process, hymen fusion occurs.

Repetition of this process, in the case of a normally formed hymen, is often associated with childhood illnesses such as diphtheria, scarlet fever, measles and others. It can also be caused by various vulvovaginitis, adhesions and scarring.

Causes

In most cases, the pathology is congenital. It is observed much less often with a correctly formed hymen.

Congenital atresia

Disturbances in the structure of the hymen occur during the period of intrauterine formation of organs, namely at the 19th week of pregnancy. At this stage of gestation, any negative factor can cause an anomaly, which is sometimes discovered only at the beginning of the girl’s puberty.

Secondary atresia

Less common is acquired anomaly of the hymen. The causes are diseases suffered at an early age:

  • measles;
  • scarlet fever;
  • diphtheria;
  • vulvovaginitis.

Consolidation in the mucous membrane is also caused by scars and adhesions formed after surgery.

Symptoms of this pathological process

In most cases, in girls, before the onset of puberty, this fusion does not manifest itself clinically. There are very rare cases when patients at this age complain of mucocolpos or hydrocolpos, which can be caused by accumulated secretions, putting pressure on neighboring organs and causing disorders of the urinary process. Typically, the symptoms of this pathology begin to manifest themselves with the onset of menstruation. Girls, in most cases, begin to feel recurring pain in the lower abdomen and in some parts of the lower back. Due to the fact that menstrual blood cannot find a way out, it begins to accumulate in the vagina, which is closed by a dense hymen, and begins to stretch its walls. As a result of this process, hematocolpos is formed. If no action is taken to eliminate this condition, the process of blood accumulation may continue in the uterine tubes, which will lead to the occurrence of hematosalpinx, as well as in the uterine cavity, which will lead to the occurrence of hematometra.

Hymenoplasty technique

Typically, surgical defloration is performed under local anesthesia. If the vaginal septum is too dense, then general anesthesia can be used.

It is worth noting right away that this operation requires only a few minutes and that it is not a complex operation.

Her technique is quite simple: the hymen is cut in several places with a scalpel, resulting in minor bleeding. There's no point in stopping him. Finally, a tampon is placed, and after an hour you can safely leave the clinic, unless, of course, general anesthesia was used. With general anesthesia, of course, you will have to rest for some time.

Symptoms

Signs of hymen atresia appear during puberty. They are not observed in girls under the age of 12-13 years. During the first menstruation, blood accumulates, puts pressure on the ovaries and the walls of the uterus, and provokes:

  • general malaise;
  • increased body temperature;
  • pain in the lower abdomen, radiating to the lower back;
  • feeling of fullness;
  • frequent urination.

The last two symptoms are caused by vaginal and blood accumulations affecting the organs of the genitourinary system. False amenorrhea is another sign of pathology.

If you complain about one of the symptoms, you should consult a gynecologist. Timely treatment will prevent the development of infectious diseases!

Preparation for surgery to remove the hymen - hymenotomy

Hymenotomy, although a relatively easy procedure, requires proper preparation.

Specific tests to perform:

  • blood group test;
  • blood sugar test;
  • general urine analysis.

The woman will also be advised to stop taking certain medications that may increase bleeding during and after surgery (eg, aspirin, ibuprofen, vitamin E). Quitting smoking and drinking alcohol is also important for the proper healing process.

The procedure is usually performed in the first phase of the cycle after the end of menstruation. 6 hours before the hymenotomy you should not drink or eat.

How much does the operation cost?

The price will vary depending on the complexity of the operation, the woman’s health condition, the type of anesthesia she chooses (agreed with the anesthesiologist), and the qualifications of the surgeon.

In Moscow, minimum prices start at 1000 rubles, 1500 rubles and higher. The maximum prices that can be found in some clinics sound quite impressive - 15,000 and 17,000 rubles. But the average surgical defloration price, combined with an assessment of the good quality of diagnosis and treatment in Moscow, ranges from 3 to 6 thousand rubles.
