Mesobotox is a procedure for introducing low concentrations of botulinum toxin, carried out using mesotherapy injection techniques into the upper layers of the dermis. The technique was first described by Bernard Hertzog, and has the following main areas of application. Firstly, if there are many small wrinkles in the eye area (especially along the lower edge of the orbit, where botulinum toxin injections in standard dilution cannot be done), secondly, if there is a network of small wrinkles in the lips or cheeks. A separate indication is for the treatment of acne in the forehead area.
As we have already said, the use of the prefix meso- in the term “mesobotox” indicates that the injections are placed in the mesoderm (the superficial layer of the dermis from which the dermis develops, fibroblasts originate, and into which multiple very thin superficial muscles are woven). Injections are made superficially in a checkerboard pattern, into small subcutaneous papules. The standard dosage for Botox® is 10-15 units, which are diluted in 0.7-1.0 ml of saline solution.

The administration of Botox provokes a disruption of neuromuscular conduction, temporarily blocking the flow of nerve signals from muscles or glands. The drug is administered subcutaneously or intramuscularly, after preliminary consultation with a doctor and a thorough examination of the injection area. Part of the botulinum toxin is inactivated, the effect will be less pronounced. Due to these same factors, botulinum toxin can spread beyond the target muscles and nerves, and immobilize fibers that should not be used. This can lead to unexpected results, especially around the eyes. It is advisable to do the injections in the first half of the day, after which you can take a quiet walk or sit, without bending over or taking a horizontal position.
Alcohol consumption
Ethyl alcohol leads to vasodilation and increased capillary blood flow, a rush of blood to the face. Therefore, it is possible to partially neutralize Botox or distribute it to undesirable areas. In addition, drinking alcohol reduces the effectiveness of control over one’s actions, provokes drowsiness and non-compliance with other rules of rehabilitation (lying on the couch, rubbing the face, activity, bending over).

What to do after Botox injections

Let us recall that acne is caused by a number of factors, including: 1) increased activity of the sebaceous glands, accompanied by an increase in sebum production, 2) hyperkeratosis of the epithelium of the hair follicles, leading to blockage of their lumen, 3) colonization of the follicles by Propionibacterium acnes bacteria, 4) inflammation follicles and dermis. In addition, in adolescence, these processes are influenced by an increase in the concentration of androgens circulating in the blood, which can also increase the activity of the sebaceous glands.
An interesting effect of botulinum toxins is that they can inhibit the cascade of these processes in patients with acne. The effect of botulinum toxin in this case develops through different mechanisms. For example, due to its effect on the parasympathetic nervous system, botulinum toxin can reduce the activity of the sebaceous and sweat glands, as well as promote faster renewal of keratinocytes in the epidermis. Botulinum toxin type A is able to inhibit the formation of acne elements through at least 3 mechanisms. All this helps to inhibit the growth of P. acnes bacteria. A decrease in the activity of the sebaceous glands also leads to an increase in the concentration of linolenic acid in the sebum they secrete. This acid prevents the development of hyperkeratosis of the epithelium of the hair follicles and thus prevents blockage of the follicles.
Secondly, by inhibiting the release of acetylcholine, botulinum toxins also promote the renewal and migration of keratinocytes, which also reduces the risk of blockage of hair follicles. Well, the last point - we have already said above that the androgenic hormonal background can greatly influence the development of follicular occlusion (i.e. when the lumen of the hair follicles is blocked by the secretion of the sebaceous glands and desquamated epithelial cells). This is due to the fact that androgen receptors are present on keratinocytes along the ducts of the sebaceous glands, and botulinum toxin has an antiandrogenic effect, reducing the number of such receptors.

If we take Botox®, then to treat one area (for example, forehead or cheeks) 10-15 units diluted in 0.7-1.0 ml of saline are enough. Injections are made into the middle layers of the dermis, in small papules in a checkerboard pattern. At the same time, during injections, you should never injure acne elements with a needle. The needle bevel is facing upward, the needle diameter is 32G. On average, the effect will last from 2 to 3 months. Repeated courses should be carried out no more often than once every 3 months.
