There were no doctors in our family; I grew up in a family of officials and party workers. At school I did not like exact sciences. I wanted something humanitarian, but, for example, I had no special desire to become a biologist either. At the same time, it seemed that medicine could easily captivate me. When I entered medical school, I realized that I would definitely not be a therapist, but that I should try to become a surgeon. I tried. And when I became one, I soon learned what a hard man’s job it was.

We started in the 80s. At that time in our country there was no trace of such a specialty as “plastic surgeon”. Officials believed that this was a “capitalist direction in surgery,” which we did not need at all. However, I was included in the first study group recruited at the Academy of Medical Sciences.

When I started doing plastic surgery, I wouldn't go home for weeks. Operations were carried out mainly at night. Just as you’re about to leave, they’re bringing in another patient. That's it until the morning. And the next day you do planned operations. In the evening something urgent happened again. And when I asked my son: “Do you want to be a doctor?”, he answered: “God forbid.” The son is already an adult, he is 24, but the daughter is 6 years old. She will no longer see those negative aspects of my work and, perhaps, will want to follow in her father’s footsteps...

We were the first to perform many operations in Russia: liposuction, artificial hair transplantation, gender reassignment, augmentation phalloplasty, replantation, facial rejuvenation using a non-surgical method. We had practically no equals. And over the past 10 years, we have accumulated a lot of experience and written a huge number of scientific papers. But the specialty “plastic surgeon” still does not exist in official surgery. Officials have not yet said “yes”; they are still looking closely at us. Come on, we’re not complaining, the main thing is that they don’t prohibit us from working.

— I read on the Internet that among doctors, surgeons are subject to the greatest stress. — According to statistical European data, firstly, anesthesiologists are exposed to stress. Surgeons are in second place. You're actually under a lot of stress. The operation is underway, everything seems to be fine, and suddenly the patient begins to bleed heavily. You could lose someone. Or during plastic surgery a person’s heart stops.

— ? - No, there were no deaths, in such situations I did an open heart massage, and everything worked out. This is just stress. And in our Russian medicine there is another category of stress. There is much more from any commission or inspection than from the most complex operation. Your fellow journalists often ask me the question: “Why don’t you go abroad with such experience and knowledge?” Do you know why? Here I am a Doctor of Science, professor, academician of countless academies, but obtaining such status abroad is quite problematic. As for plastic surgery, in the West this is an area of ​​fierce competition, it’s a lot of money. Of course, you can leave, there are invitations, but you will have to start all over again. And the years go by, I’m no longer 20, but 46.

I have been in medicine for more than 20 years, I have been practicing plastic surgery for 12 years, I have studied a lot in different countries. I grew up with a huge amount of operational material. All the operations that we are currently doing were essentially mastered and implemented by me and my colleagues. And I achieved all this, I repeat, in Russia. Today it is difficult to surprise me with any innovations supposedly born in the West. We do everything that has been tested in world practice here in Chelyabinsk, in the department of plastic reconstructive microsurgery of the regional hospital.

- In my opinion, there is something to be proud of. — Several years ago the press wrote about a sensation, saying that a torn phallus had been sewn back on in America. Surgeons from all over the New World gathered and sewed them on. And by that time we had already completed four replantations of the phallus. But the American case was presented as unique, but they wrote very sparingly about us in the local newspaper, then in the Moscow one, reducing the text to the minimum. And when I tell my Italian colleague that in four years we have done about a thousand replantations, he is sincerely surprised.

One of our unique operations was the replantation of both arms. If this were done, for example, in England, then surgeons throughout the country would be involved. And our patient was brought to the department, Pukhov and company gathered, took him and sewed him up. But the main thing is that both hands took root, and the patient then juggled weights that weighed pounds.

- Of course, what you just talked about is from the category of science fiction. And this is not happening somewhere in the capitals, but here, in the provinces. So, in the Urals plastic surgery is not marking time? — Previously, various medical societies existed here and conferences were held. Now I know what is happening, say, in Paris, but I have no idea what is going on next door, in some Chelyabinsk hospital. I participate in congresses on plastic surgery: Russian, European, world levels. I received so many certificates there that there is not enough space in the office to hang everything. And I haven’t been to one (!) in my hometown over the past few years. If something is being held somewhere, then I am not invited. What this is connected with is a question for the organizers. Before, when we just started doing plastic surgery, we walked under the banner of the region, we were proud of us, but now I don’t even know under whose banner we are?

“But they are still proud of you today.” They say our Pukhov has special hands. — A surgeon, like a chess player, first thinks through all possible combinations in his head, and then uses his hands. And, of course, something must be from God.

- Do you believe in God? “I’m a surgeon, a realist, but I still think there’s something there.” Something or someone from above is guiding us. Sometimes you perform an operation, and it seems - no, it won’t work. But this something or this someone seems to force me to continue. And, indeed, it works. Although surgeons should always rely only on their professionalism. Anything else is not possible. I am once again convinced of this by communicating with such luminaries as Professors Vazhenin and Gantsev.

— Alexander Grigorievich, people who know you told me long before the interview that you are unlikely to expand in detail on topics other than medical ones. — I believe that a doctor is like a priest. He must be private and cannot be like, for example, a representative of show business: tell what underwear he wears - from Trussardi or Hugo Boss, or something else. A doctor is a rather specific profession. People share with him things that they would never tell anyone else. Ask anyone what kind of occupation a person comes first in terms of social significance and necessity. I assure you, he will answer: a doctor or a teacher. The medical profession among the intelligentsia, especially in the provinces, is the most revered, no matter what they say. And my position is this: a doctor should be discreet and even a little mysterious... Therefore, I won’t say exactly how many fans I have (laughs).

— Consequently, you have to play a certain role in life? “I can’t always be who I want.” Patients are different, and I cannot say a rude word to someone who annoys me. I must be correct with everyone, even if something doesn’t suit me. There's probably some kind of game in this.

- So you are alone at home and another at work? — Communication among medical workers is subject to certain rules. Here you can’t wave your fists or shout, so I try to be restrained in everything. Although, of course, it happens that emotions overwhelm you to the brim, especially during surgery. When you see that it doesn’t work out due to the sluggishness of, say, an assistant. But, in general, by nature I am a calm, balanced person. I'm more of a sanguine type. Moreover, according to my horoscope I am Virgo. And this is even better. Virgos are consistent, logical, and fair. Work for them is king and God.

- Capricorn, this is my sign, according to all stellar forecasts, has good contact with Virgo... - What, do we have bad contact?

- I didn’t finish the sentence - in a marriage union. — It’s hard for a normal person, not a doctor, to live with a surgeon. Such family marriages often end sadly. It's hard to understand why, but even in a family of doctors, one of whom is a surgeon, problems often brew. A young doctor got a job in our department. Some time passes, and I see that he became somewhat sad and hung his head. It turned out that the wife, also a doctor, is dissatisfied with the fact that her husband comes home at 7 pm and at 8 pm. She is either jealous or something else. I had to talk to her and explain that this is how we all work.

- And how did the story end? “Now he walks and smiles, which means everything is fine.”

— Alexander Grigorievich, do you like music? — I really love Sarah Brighton, I enjoy listening to remixes of American artists with classical musical inserts. I like the songs performed by Valery Meladze. When you are driving in the car to or from surgery, you want to listen to calm music. In general, it all depends on the mood.

— Are your cultural preferences limited to this? — Lately, yes. When I was studying and visiting abroad, I went to various exhibitions, concerts, and theaters. But recently I got to the Bolshoi Theater, Elena Obraztsova , after the performance I went backstage, saw another life, her colleagues in the shop and...

-Have you talked to them? - No. They bore me. They have a completely different life, a lot of pretense. I don't like these companies. True, I had to visit Viktyuk with Obraztsova several times. I have the impression that they left the stage and are still performing the play. With us everything is somehow different.

- And how are you? - More down to earth.

— Do you drink vodka and talk about life? — I can’t boast that I drink vodka. I'm not a very sociable person, I have a fairly narrow circle of friends, and I don't like noisy feasts. Among my good friends are both work colleagues and people who have nothing to do with medicine. And the conversations are not only about work, about surgery. About everything. I like to spend time outdoors and exercise. I have always been interested in sports, there were many categories: volleyball, basketball, wrestling, table tennis. Of course, there is not enough time for sports now, but yes, for physical education. And relaxing on drinking and partying is not for me.

- Alexander Grigorievich, don’t take it as a flattering compliment, but you look great. - Thank you.

- Please. In my opinion, you are a great fashionista. Do you choose your own wardrobe? “I don’t trust this matter myself or anyone else.”

— Do you feel like you attach considerable importance to your appearance? — I take care of myself, correct something if it is in my power. And I think that everyone should do this. If a person takes care of himself, this is an indicator of culture.

— You mentioned Elena Vasilyevna Obraztsova several times in our conversation. What connects you? — Five years ago, Elena Malysheva, the host of the TV program “Health,” introduced us. Elena Vasilyevna Obraztsova was going to turn to a famous Spanish plastic surgeon for medical help. We met in Moscow, talked for only half an hour, and the next day she returned her ticket to Madrid and flew to Chelyabinsk. Since then, we have been meeting constantly both on business and for no reason. We are friends.

- And for the sake of Elena Vasilievna, are you ready to go to another city to see her play, to meet with actors who are tiring for you? - Yes, and drink alcoholic drinks with them, say something... She’s great, you can’t refuse her. In every room she has packed suitcases for trips to different countries: Japan, Spain, America. So that at any time, as if in an emergency, you can grab what you need and run to the plane. She has great efficiency and a lot of energy.

When we meet, she immediately drags me to the theater, to rehearsals. It’s hard and easy with her at the same time. There is no peace with her: you have to go somewhere, talk about something. She is a very talented person. She told me that she had never written poetry, but about five years ago something happened - her soul turned upside down, and she became interested in poetry. He writes on various pieces of paper that come to hand, on napkins and is going to publish a collection of his poems.

Lenochka considers herself a man from God, therefore she cannot wear a hat, she says that the headdress interrupts her higher connection. If a person is talented, then he is talented in everything.

—You don’t consider yourself like that? - No.

- But there are people who think exactly like that. Here you have a photograph signed by Elena Malysheva: “...To Alexander the Great...” - Someone may think that Pukhov is great, but this is not serious, you understand.

—Are you a strict leader? — Our relationships in the team are not very typical. At the time when I was brought up, there were teachers and professors who walked around with a “butterfly” and a cane, in galoshes and hats. And at the same time they spoke with a French accent. One of them boiled chicken in four waters, did not know English and was surprised: “What kind of English is this? My governesses spoke French and German, but what is this?” Then a very strict chain of command was observed, and I was brought up in strictness. And since we started almost everything in our team together, we have more loyal relationships.

— Your subordinates don’t use this? - Sometimes, of course, they take advantage, and then I can be quite strict. But in general, the relationship can be said to be friendly. With peace of mind I can leave the department and patients to them. I trust my colleagues, candidates of medical sciences Medvedev, Popugaev, Nazhny.

— Did you, Alexander Grigorievich, fall in love with your patients? - Well, I like some, but to fall in love... I don’t even remember.

- In that case, how can a woman attract your attention? - I won’t lie. Just as a designer pays attention to the elements of clothing and interior that interest him, so I see in a person something that I can correct.

- So, a priori, you cannot like a woman? Or do you have an ideal of beauty? — I can’t say that I might like a tall brunette or vice versa. But I notice female beauty. You can catch something in a glance and you will never forget it, and there is incredible charm. Beauty comes in different forms, but we also create it.

— Do you think that you have achieved great success in this? - I don't think so. Yes, a lot is working out, yes, in many ways we have no equal, but I have not yet achieved success with a capital S. Years have passed in work, but everything is still ahead and we cannot stop there.

— Are there new goals and tasks ahead? - Yes, there is one grandiose plan, but I won’t say what it is yet. Secret.

—Are you afraid of jinxing it? - Yes, although I’m generally not a superstitious person and I don’t particularly believe in omens, but...

- Is the patient always right? — People perceive and understand the outcome of operations differently. In general, in plastic surgery you have to talk with the patient for a very long time, explain in detail what is and what will happen. Some patients come to the department every day after surgery and show that there are fewer wrinkles, but there are two wrinkles on one side and three on the other. I look and see nothing. I have to take special magnifying glasses, and... again I can’t see anything. One patient complained to us that her husband, after she had plastic surgery, never returned to her, and other men did not turn around. She promised big trouble. There are simply inoperable cases when surgery has to be refused, and the patient is again not happy. But often people simply leave words of gratitude in our memorial album, and one patient wrote: “...Pukhov, I almost love you...” Or such a case. The lady told us that according to the lunar calendar, the operation should be performed on the 23rd in the afternoon. She wants it that way and we do it that way. There have always been and always will be satisfied and dissatisfied patients. But our attitude towards the patient is fair and objective. There is only one strict rule: women and children cannot be denied operations.

— Alexander Grigorievich, do you have any students? — I consider the surgeons of our department to be my students. True, there are students in other cities of Russia and abroad. Under my guidance, they write dissertations and defend them. On the other hand, I myself continue to study. There are also numerous contacts with foreign colleagues, communication, meetings and joint practical work. I’ve been in medicine for 20 years already, so much has been done, but it seems like you know little, and there’s still more to come.

— Are you interested in working with foreign colleagues? — I have been communicating with Walid Khalil for many years, he is a plastic surgeon from Paris. He and I developed phalloplasty - a method of penis enlargement, and he defended his Ph.D. thesis on this material.

- Actual topic? - In Paris - yes. I participated in many operations with Khalil. It is interesting that the patient is given an anesthetic injection, during the operation he is conscious and takes an active part in it with his wishes.

— Do our men pay the same close attention to their “advantages”? - It's all about mentality. If a Russian person is more concerned about what to eat and drink, then a French person is more concerned about something else. They carry out such operations every day, but for us - at most one or two a month. And, in general, the beauty of the face and body is, unfortunately, still a cult only in European countries.

— Listening to you, do you come to the conclusion that everything in your life is going according to plan? “It’s not as smooth as it might seem.” And it’s not even about stress or complex operations. We still have to fight with officials. Until now, they do not want to officially admit that plastic surgery was, is and will be. One Moscow official told me: “You are using prohibited methods!” He doesn’t even know that our methods are already permitted not only de facto, but also de jure. I think in this case there is a feeling of professional envy: how is it that he became a doctor of science, and I work two floors above and am not even a candidate?

- But awards and titles are a kind of body armor that protects envious people from the stinging arrows. - Perhaps, but I am beyond awards. I have titles - that's understandable. And it happens that diplomas and certificates come from abroad. One such surprise for me was a letter from the American Biographical Institute. Overseas scientists awarded me the 2000 Presidential Honor Award. The document says that “this is a tribute to the nation and recognition of accomplished deeds.” By and large, all these certificates and diplomas are rather for the quiet satisfaction of healthy personal ambitions.

— At the very beginning of our conversation, you emphasized that as a child you did not dream of being a doctor. Now, years later, can you say with confidence that you have chosen the right path in life? Or could they have realized themselves more fully in something else? - I became a doctor. And that means my mission is to heal and save people all my life, to restore beauty and health to them. There is no turning back.
