Department of Reconstructive and Plastic Surgery

Almost 45 years ago, in 1973, at the All-Union Scientific Research Institute of Clinical and Experimental Surgery of the USSR Ministry of Health, the director of the institute, academician B.V. Petrovsky issued Order No. 99 of July 23, 1973: “To accelerate the development of the problem of vascular microsurgery, to form a research group on vascular microsurgery, consisting of two senior researchers, two junior researchers and one laboratory assistant, on the basis of Moscow City Hospital No. 51.” This is how microsurgery began in our country, and a process began that later led to the formation of the department in the form in which it exists now. Undoubtedly, the founders of microsurgery in our country were Academician B.V. Petrovsky and the first two senior researchers of the vascular microsurgery group - V.S. Krylov and G.A. Stepanov.

The work of the vascular microsurgery group in 1973 began with the acquisition of microsurgical equipment and the first introduction of microsurgical equipment into the clinic when performing reconstructive operations on the vessels of the extremities. In 1975, the vascular microsurgery group was replenished with two junior researchers - R.S. Akchurin and N.O. Milanov. In the same year, by order of the director of the VNIIK and EH of the USSR Ministry of Health No. 81 dated April 30, 1975, the group of vascular microsurgery was transformed into the department of vascular microsurgery.

Mastering microsurgical techniques in experiments made it possible already in 1975 to begin solving the clinical problems of replantation of fingers and hands amputated as a result of injury, and in April 1976 to obtain the first engraftment of the replanted first finger of the hand. In the same year, for the first time, a hand amputated at the level of the wrist joint was successfully replanted; in 1978, success was achieved with the simultaneous replantation of two hands; and in 1981, the upper limb was successfully replanted at the level of the shoulder. In 1978, the first successful microsurgical autotransplantation of a complex composite flap was performed. And in 1979, the first successful transplant of a big toe was performed to replace a big toe lost as a result of injury. By 1980, two main directions in the development of microsurgery were clearly defined - replantation of fingers, hands and larger segments of extremities and autotransplantation of free revascularized tissue complexes, which led to the publication of an order by the director of the All-Union Scientific Center of Surgery of the USSR Academy of Medical Sciences B.V. Petrovsky No. 8 of January 28, 1980: “In accordance with the Order of the First Deputy Minister of Health of the USSR S.P. Burenkov No. 900 dated August 29, 1979 “On the organization of the All-Union Scientific Center for Surgery in Moscow” and Resolution of the Council of Ministers of the USSR “791 dated August 10, 1979: “... to form a department of microsurgery as part of the department of microsurgery and the department of emergency microsurgery.”

In September 1980, the microsurgery department first appeared in the center’s staffing table. Professor V.S. Krylov was appointed head of the department. It included: the department of microsurgery - the head of the department and department, Professor V.S. Krylov, senior researcher N.O. Milanov and junior researcher A.M. Borovikov, and the department of emergency microsurgery - head d.m.s. G.A. Stepanov, senior researcher R.S. Akchurin, senior researcher Yu.A. Peterenko and two junior researchers V.A. Rudolphi and V.V. Denisov. In 1982, a group of scientists consisting of: V.S. Krylov, G.A. Stepanov, R.S. Achkurin and N.O. Milanov (Moscow), L.V. Lebedev, V.N. Vavilov and G.N. Gorbunov (Leningrad), G.A. Natsvlishvili and I.E. Kuzanov (Georgia, Tbilisi) and N.F. Dryuk (Ukraine, Kyiv), for the development of problems of microsurgical replantation of fingers during traumatic amputation, was awarded the USSR State Prize. In 1986, a group of young employees of the microsurgery department of the All-Russian Research Center of Surgery of the USSR Academy of Medical Sciences was awarded the Lenin Komsomol Prize. In 1989, the experience of the microsurgery department of the All-Russian Research Center of the USSR Academy of Medical Sciences in replantation of limb segments and free microsurgical autotransplantation of revascularized tissues, the experience of one department, one hands, was recognized as the greatest experience in the whole world.

In 1986, N.O. became the head of the department of planned microsurgery of the All-Russian Scientific Center for Surgery of the Russian Academy of Medical Sciences. Milanov, under his leadership in 1988 the Department of Plastic and Reconstructive Microsurgery was created, which was later called the “Department of Reconstructive Surgery and Microsurgery.” In 1999, after the restructuring of the department, two departments were created within the latter: the department of plastic and reconstructive microsurgery (headed by E.I. Trofimov) and the department of microsurgical correction of sexual disorders (later - the department of microsurgical autotransplantation of tissues, headed by R.T. Adamyan) .

At the beginning of 2020, the Department of Reconstructive and Plastic Surgery was headed by Professor, Doctor of Medical Sciences R.T. Adamyan.

microsurgical operations to eliminate defects in the bones of the facial skeleton with subsequent rehabilitation of the function of chewing, swallowing, speech, correction of facial and bite deformities;
reconstructive operations for injuries and post-traumatic deformities of the zygomatico-nasal-orbital complex: reconstruction of the upper, lower, outer and inner walls of the orbit using bone autografts, preparation of the orbit for endo- and ectoprosthetics, osteoplastic surgery on the facial skeleton in case of violations of facial proportions;
surgical operations on the temporal mandibular joint;
treatment of fractures of the facial skeleton using modern methods of osteosynthesis;
removal of benign neoplasms of the craniomaxillofacial area with simultaneous tissue restoration of any category of complexity;
operations to change the shape of the nose (rhinoplasty) and deformations of the nasal septum (rhinoseptoplasty);
dento-alveolar surgery: restoration of the alveolar process of the upper and lower jaws and dental implantation; extraoral implantation for silicone prostheses;
harmonization of appearance, orthognathic surgery, osteotomy of the upper and lower jaw, genioplasty
surgical dentistry: removal of teeth of any complexity;
aesthetic surgeries to eliminate deformities of the nose, ears, cheekbones, neck, chest, abdomen;
aesthetic surgeries to correct age-related changes (facelift, mammoplasty, blephoroplasty, rhinoplasty, etc.)
