Olesya Andryushchenko: Upper eyelid surgery is more in demand by patients, and in my practice I perform it much more often than lower or circular blepharoplasty. But this plastic surgery of the upper eyelids does not make it any easier. The main nuance is to remove the exact amount of tissue: no more, no less, and position the scar exactly in the fold to camouflage it as much as possible. And this is always an individual approach, since each patient’s facial anatomy is special and it is important to take into account all the nuances of the structure: the shape of the eyes, the location of the eyebrows, the size of the skin fold, etc. Sometimes, for example, due to low sagging eyebrows, I suggest that the patient combine upper blepharoplasty with an endoscopic forehead lift. The operation is not traumatic with short rehabilitation, and in this combination we achieve maximum rejuvenating results. As you can see, upper eyelid surgery also requires maximum professionalism and concentration from the plastic surgeon.

Corr.: In what cases do you recommend patients undergo lower or circumferential eyelid surgery?

Olesya Andryushchenko: A very interesting question. There are two diametrically opposed indications in lower eyelid surgery. The first and most common are “bags” or fatty hernias under the eyes. They give the face a puffy appearance and visually make it look older. The second indication, on the contrary, is skeletonization of the periorbital region. Patients with this problem have a gaunt, tired appearance.

If, with a drooping upper eyelid, I diagnose a patient with one of the problems described above, I recommend undergoing circumferential eyelid surgery. Simultaneous rejuvenation of the upper and lower eyelids gives excellent results.

Corr.: How is lower eyelid surgery performed for the described problems?

Olesya Andryushchenko: In the first case, I remove fatty hernias or redistribute them (the so-called fat-saving blepharoplasty). In the second, the strategy is different: it is necessary to lift the sagging muscle and again add lipofilling to the lower eyelid area. Since the eyelash margin is the thinnest and most delicate area, lower blepharoplasty should be trusted exclusively to an experienced surgeon.

Corr.: Is fat-saving blepharoplasty your new technique?

Olesya Andryushchenko : Yes, this is a relatively new technique for performing the operation. With age, the skin of the lower eyelids loses its elasticity, and the volume of fatty tissue on the face decreases, the eyelid area becomes thinner, and the fat itself migrates or becomes deformed, forming so-called “bags.” In this case, I combine eyelid surgery with lipofilling: I delicately remove excess sagging skin, and evenly redistribute fatty hernias in the area of ​​the nasolacrimal groove and lower eyelids. If necessary, we add fat to the cheekbones from donor areas (abdomen or thighs). The operation allows you to get rid of sagging skin, bags and bruises under the eyes, as well as create a smooth transition of the lower eyelid to the cheekbones,

The main indications for fat-saving blepharoplasty are deep nasolacrimal grooves, excessively loose skin, and “sunken” eyes. By the way, not only older patients come to me with such problems, but also girls 28-30 years old.

Corr.: What does a mid-zone lift through the lower eyelid mean?

Olesya Andryushchenko: This operation is called check-lifting. This method is effective when age-related changes in the eyelids are accompanied by sagging of the midface. Thanks to check-lifting, you can get not only aesthetically attractive eyelids, but also clearer cheekbones, a smooth cheek contour and a significant reduction in nasolabial folds.

During the operation, I use access through the lower eyelids in order to partially isolate the tissues of the middle zone, move them upward to their original “young” position and fix them. As a rule, check-lifting of the middle zone becomes relevant after 35 years.

Corr.: Why does ectropion of the eyelids occur and how often does this happen after eyelid surgery?

Olesya Andryushchenko: The cause of eversion of the eyelids is excessive removal of skin in this area. The specialist needs to know the extent of surgical intervention. And to do this, you should always accurately assess the current condition of the skin and, I repeat, the anatomy of the face. It is possible to fix the problem with repeated blepharoplasty, but it is much more difficult than if everything is calculated correctly the first time.

Therefore, I recommend a critical and conscious approach to choosing a surgeon: pay attention to his experience, photos of his work, certificates, and how he behaves during a consultation—whether he offers alternative methods. And, of course, study this issue in detail yourself. Come to your consultation with a specialist as prepared as possible.

Corr.: How long does rehabilitation last after eyelid surgery?

Olesya Andryushchenko: For the first 5-7 days, swelling may persist, hematomas and tearing are possible. Don't panic, this will all pass in a few days. Most of my patients return to work within a week after eyelid surgery. The final result of the operation can be assessed after a month.
