Pharmacies and beauty salons offer various ointments, creams, and gels for acne. They differ in composition, price and effectiveness. It is often difficult to choose what is really needed. Which anti-acne ointment should I take – retinoic or salicylic? How effective will the chosen ointment be for large acne? And how long can you use hormonal drugs?

Let's try to figure this issue out. Let's consider what modern pharmaceuticals offer, and which ointments help with acne on the face and which do not .

To do this, we will analyze the composition of each ointment, the features of its action and recommendations for use - when it is really effective, and when you can do without it. Let's start with two well-known ointments that were used in Soviet times to treat any boils and abscesses - Vishnevsky and Ichthyolka.

Ichthyol ointment for acne

Ichthyol ointment contains ichthyol

or
ingammol
- an extract from shale rock resins.
The main advantage of this ointment is that it draws out pus from under the skin .
A targeted application of ichthyol allows you to form an abscess, draw purulent contents onto its surface, after which it is removed and treated with an antiseptic. It is necessary to apply ichthyol if the abscess stretches for a very long time and hurts . Typically, such a pimple is quite large in size and looks painful and compacted. Without ichthyol ointment, it can ripen for up to 2 weeks, and “break through” for the same long time, and only then can it heal.

In addition, ichthyol is used as an alternative method of cosmetic facial cleansing. It dissolves comedones (blackheads, blackheads), which are oxidized fat in the pores of the skin. And they stimulate the spontaneous flow of sebaceous secretions, cleaning the pores of blackheads.

Note: after all, cleaning (squeezing) acne from a cosmetologist is more effective. Therefore, it is worth remembering that ichthyol is an ointment for subcutaneous acne, and not for acne.

Squeezing pimples and blackheads (comedone extraction)

For comedonal and papulopustular forms of acne, invasive methods are sometimes used, for example, comedonal extraction. Extraction means their removal/squeezing out - using special comedones extractor tools, or by palpation. Moreover, we are talking about the removal of macrocomedones, i.e. large eels. But squeezing out pimples (papules and pustules) can, at best, lead to the appearance of a hyperpigmented spot in this area in the future.

In the worst case, self-squeezing pimples and blackheads can lead to the spread of inflammation and the formation of deep subcutaneous pimples, which will leave superficial or deep atrophic scars on the skin. Therefore, only comedones (acne) can be squeezed out, and secondly, this should only be done by a dermatologist or a dermato-cosmetologist. The procedure is called “mechanical facial cleansing.”

Important: keep in mind that the removal of comedones is much easier and without complications if the patient has previously used topical retinoids for at least 3-4 weeks. The fact is that topical retinoids (adapalene) also have comedonolytic properties.

Vishnevsky ointment for acne

Vishnevsky's composition has the same indications as ichthyol ointment. It is also used to draw out ulcers . Vishnevsky's ointment contains natural ingredients ( birch tar, castor oil

) and
the antibiotic xeroform
. What is the difference between the two ointment formulations?

Ichthyol - accelerates the maturation of the abscess. It draws out pus from the superficial subcutaneous layers. Vishnevsky ointment - makes the abscess more extensive in order to draw out pus from the deeper layers of tissue. Thus, Vishnevsky ointment removes pus completely. Hence the following recommendations: first use ichthyol ointment for acne on the face, and after the abscess breaks out, use Vishnevsky’s composition.

Note: before the discovery of antibiotics, the main means for treating skin inflammation were two ointments - based on ichthyol (Ichthyol) and ointment with xeroform (Vishnevsky). Today these are the two most inexpensive ointments for acne.

Salicylic ointment for acne

Salicylic ointment - contains a powerful keratolytic agent ( salicylic acid

). The action of the acid is so aggressive that it is used to soften keratinized skin seals (calluses). When applied to the skin as part of an ointment, salicyl has an exfoliating effect. It removes old dead cells and thereby exfoliates the face.

In addition to peeling, salicylic ointment is used to treat skin infections - acne and rashes. It dries, so it is good for oily, shiny skin . And not suitable for dry, flaky epidermis (increases the feeling of dryness).

Recommendations: salicylic ointment contains an aggressive component (in the amount 1%

). Therefore, it is worth using it for obvious redness and inflammation on oily skin.

Note: due to the effect of exfoliating old cells, salicylic ointment is used as an ointment for acne scars. It causes a burning sensation and accelerates local blood flow, and this ensures regeneration (renewal) of skin epidermal cells.

Phototherapy and lasers for acne treatment

Phototherapy and lasers are used to treat acne - either endogenous porphyrins contained in P. acnes, or sources of exogenous porphyrins are additionally applied to the skin before the procedure. Porphyrins have the property of absorbing light in the blue regions of the spectrum (wavelength 415 nm), which leads to the formation of unstable oxygen and, as a result, to the destruction of pathogenic bacteria P.acnes. The FDA recommends Lumenis devices (for example, the IPL module of the M-22 device), which use high-intensity light from the blue part of the spectrum, for the treatment of papulopustular acne, i.e. acne.

Light from the red part of the spectrum can also be useful because... Compared to blue light, it has a stronger anti-inflammatory effect (although it has a lesser effect on porphyrins). Therefore, a combination of light from the blue and red parts of the spectrum will be even more useful. Facial procedures are carried out 2 times a week for 15 minutes. According to the dermatology textbook Fitzpatrick's Dermatology, after a 4-week course of therapy in 80% of patients, the number of acne decreased by 60%. But in the absence of supporting procedures, their number returns to the original level - within 3-6 months.

Photodynamic therapy is even more effective in treating acne. It consists of applying exogenous porphyrins to the skin 1 hour before the procedure (aminolevulinic acid/ALA), after which exposure to a high-power light source occurs. The latter can be pulsed dye lasers, intense pulsed light or a broadband light source in the red part of the spectrum. ALA is metabolized in the sebaceous glands and hair follicle to porphyrins, which when exposed to light lead to the release of unstable oxygen, which damages the sebaceous glands. Several photodynamic therapy treatments can provide improvement for up to 5 months.

Pulsed KTP laser (wavelength 532 nm) - also leads to a reduction in the number of acne by about 40%. Using this laser 2 times a week for 2 weeks, i.e. a total of 4 procedures are required). In addition, some of the non-ablative infrared lasers at 1450 nm and 1320 nm wavelengths may also be useful for treating acne. they cause thermal damage to the sebaceous glands. However, any of these lasers requires a course of several treatments (and the improvement lasts up to about 6 months).

One of the newest acne treatment devices is the Isolaz device (from Solta Medical). The nozzle of the device is made in the form of a photo-pneumatic device, which creates negative pressure that sucks out the fatty secretion from the mouths of the hair follicles. Immediately after this, broadband pulsed light (wavelength from 400 to 1200 nm) is turned on, which acts on the P. acnes porphyrins, destroying these bacteria, and also has an anti-inflammatory effect.

What do phototherapy and lasers do?

Regular use in courses not only improves the course of acne, thereby reducing the number of inflammatory elements. The use of phototherapy (IPL) and lasers in courses often avoids the need for oral medications (antibiotics, isotretinoin), as well as the numerous side effects associated with them. Synthomycin ointment for acne

). Before the invention of the common antibiotic chloramphenicol, syntomycin was the main treatment for bronchitis, gonorrhea and syphilis. Afterwards, it was forced out of the field of treatment of internal diseases, but remained in the composition of external ointments.

Synthomycin ointment is effective if rashes and pimples are caused by a bacterial infection (staphylococcus, streptococcus). However, it is far from harmless. The antibacterial component disrupts the skin microflora. Therefore, it is not recommended to use the composition for prevention daily .

Recommendations: the ointment has proven itself well for teenage rashes . It is also used to smear the bikini area after shaving (to prevent irritation and the appearance of small pimples). And one more thing: if there is no noticeable improvement within a week of using the ointment, it means that the main cause of acne is not infectious. We need to look for another effective remedy.

Baneocin ointment for acne

We continue our acquaintance with antibacterial ointments. The list of ointment compositions is presented here in ascending order. So syntomycin is one of the weak antibiotics. Baneocin is a more potent drug. Further there will be even stronger antibacterial compositions of ointments.

Baneocin ointment contains two antibiotics that mutually enhance each other's effects. Bacitracin

– for the treatment of streptococci and staphylococci.
And neomycin
- for the treatment of Klebsiella, Shigella, the causative agent of cholera and tuberculosis infections.

Note: these antibiotics are usually not prescribed orally (due to their toxicity). Used as part of an external antiseptic ointment.

The combined use of two antibiotics expands the spectrum of activity of the drug. The ointment is intended for external treatment of skin inflammations. It is applied to purulent wounds, extensive burns, boils, as well as to small pustular rashes and pimples on the face .

Note: sometimes Baneocin is considered as an analogue of Levomekol. With the difference that the latter is produced by a Russian manufacturer and costs 2 times less than Baneocin, a product of a Swiss pharmaceutical company.

Ways to get rid of post-acne

The intensity of post-acne is directly related to the severity of acne. Therefore, it is important to work not only to eliminate the consequences, but also to prevent the appearance of new elements on the skin. It is necessary to fight acne itself, eliminating its main causes. If pimples appear again and again, the prognosis for post-acne treatment is questionable; inflammation will give rise to new scars. Therefore, work with an experienced dermatologist-cosmetologist is required.

Most of the work on restoring, smoothing the skin and eliminating scars is carried out by a cosmetologist, but certain activities can also be performed at home. Moreover, only an integrated approach will give the most maximum results.

What can you do at home?

You can also use masks with blue clay. The recipe for making a mask is very simple: three tablespoons of powder are diluted in water to a paste, applied to the problem area for 20 minutes and then thoroughly washed off with water.

What treatment methods are used in a cosmetology clinic?

Although homemade recipes help in the fight against post-acne, you should not think that only through them you can eliminate all problems. Home methods act on the very top layers of the skin and reduce only minor defects.

Modern cosmetic procedures will help get rid of deep scars, pigmentation and unevenness. But don't expect instant results.

Treatment methods are selected individually and comprehensively. The more severe the initial problem and the wider the affected area, the longer the treatment will be.

To achieve maximum smoothness of the skin and an even complexion or eliminate blemishes on the body, you need a whole series of procedures, supplemented by home care and carefully selected cosmetics. On average, the first noticeable results will be visible after 1-2 months with regular implementation of all necessary treatment measures.

If there is unevenness, decreased skin tone, pigmented or stagnant spots, peelings come to the rescue. There are a lot of treatment options: these can be mechanical peels (compositions with herbs, small dense particles) and chemical peels (with retinol, glycolic, resorcinol or other compounds).

How to consolidate the results obtained and reduce the likelihood of acne recurrence? Follow these tips:

  • During and after therapy, eat properly, do not indulge in fatty, fried, smoked and flour products. Include foods that contain zinc in your diet (beans, legumes, nuts, liver).
  • When using zinc ointment, it is advisable to refrain from using other medications for acne, as this may cause the development of various side effects or minimize the healing effect of the ointment.

Retinoic acne ointment

Its action is based on its ability to enhance blood circulation and skin cell regeneration. The manufacturer of the ointment regulates its effectiveness against acne and to prevent wrinkles, as well as as an ointment for spots after acne on the face. The effect of the ointment is ambiguous. It irritates the skin and causes slight redness, sometimes burning . There are cases where, after retinoic ointment, the skin received a “chemical” burn. However, this does not happen often.

Based on so-called retinoids, ointment compositions are produced for the treatment of complex rashes, acne and pimples. Retinoids are used when many remedies have been tried and there is no noticeable improvement. They are also prescribed to treat rosacea and nodular cystic acne .

The most aggressive formulations with retinoids - Roaccutane

and
Aknekutan
. A dermatologist prescribes them by prescription for severe acne that is difficult to treat.

Note: if the rashes on the face are persistent and stubbornly do not respond to treatment, this means that there is an internal cause (diseases of the internal organs).

Conclusions: the effect of retinoic ointment against wrinkles is not always noticeable. But its effectiveness against ordinary pustular rashes is high. In addition, the retinoic component prevents any scars and is an ointment for acne marks.

It is important to know that retinoids cause deformities in the fetus. Therefore, it is not recommended to use the ointment during pregnancy and during its planning period (when there is no pregnancy yet, but conception is already expected). After using Roaccutane, pregnancy should not occur earlier than a month later.

How to use

The use of zinc ointment for acne has its own characteristics:

  1. Wash your hands with soap and then disinfect them using vodka or alcohol. This will help prevent infection from entering the inflamed skin.
  2. Do make-up removal if you plan to apply the ointment to your face, or take a swim if you plan to apply it to your butt or back.
  3. Apply the ointment in a thin layer to the acne affected area or dotted onto each pimple for small rashes. For spot application, it is convenient to use a match or a cotton swab.
  4. You can apply zinc ointment up to 6 times a day, but it is best to do no more than 3 times so that the skin can breathe.
  5. The ointment is almost not absorbed, so 30 minutes after application you should blot it with a paper towel.

An allergy test is an important condition for using zinc ointment. Apply a little onto your wrist. After 15-20 minutes, if there is no itching and redness, then you can use the drug.

This type of therapy is not suitable for working women, since after applying and removing the ointment, the skin must be allowed to breathe; decorative cosmetics should not be applied. Experts advise using zinc ointment for acne on the butt and back, as it has a pronounced effect: it quickly removes comedones without causing dry skin. In order to cope with acne in these areas of the body, you should steam the skin at least once every 5 days, and then apply healing ointment to it. If there are few rashes, then the product is applied pointwise using a cotton swab.

Apply the product to problem areas at least five times a day. The ointment is poorly absorbed, so a quarter of an hour after application it should be blotted with a paper napkin. After using the ointment, you should not wear clothes, as the product leaves stains on them.

During pregnancy

During pregnancy and lactation, zinc ointment should be used with caution and only after permission from a doctor. Despite the fact that it can be used even in babies, hormonal levels change during pregnancy, which can cause an allergic reaction.

Akriderm ointment for acne - hormonal treatment

Akriderm ointment contains the hormone betamethasone

. The hormonal component inhibits the development of inflammation. In addition, the ointment has an antiallergic effect. It reduces atypical inflammatory reactions, and thereby reduces swelling and redness. Therefore, it is used for inflammation of an allergic nature (dermatitis, eczema, psoriasis). And also from warts, calluses, and corns.

Akriderm ointment is not used for the treatment of acne, blackheads, acne, comedones (different names for the same phenomenon are listed - acne).

Hormonal ointment for acne may be needed if, against the background of allergic redness, severe irritation appears or a bacterial infection occurs. In this case, in addition to the main treatment (antibiotic ointment), an antiallergic drug will be needed.

Antibiotics for acne treatment –

Antibacterial drugs for the treatment of acne can be used both for external use (usually in the form of gels) and in the form of tablets for oral administration. Modern research shows that antibacterial therapy in patients with acne is not very effective, because Over time, it causes the formation of resistant strains of bacteria and, as a result, leads to more severe acne. But in some cases, antibiotic therapy for acne is still necessary, and in addition, there are ways to reduce the risk of bacterial resistance to antibiotics.

a) Mild to moderate papulopustular acne:

Antibiotics, if prescribed, are only externally. These may be drugs such as Dalacin, Clindovit or Clindatop. These drugs are monocomponent gels containing the antibiotic clindamycin. In addition, there are drugs in the form of solutions for topical use, for example, the drug Zerkalin. This drug is clindamycin based, but it also contains alcohol, which is actually not very good for acne patients (especially if you are also using retinoids and benzoyl peroxide).

Another drug - Zinerit (in the form of a solution that is applied using an applicator) - is based on erythromycin, but there are now many resistant strains of the bacterium P.acnes to this antibiotic. There are also combination preparations that, in addition to the antibiotic, may additionally contain either the retinoid adapalene or benzoyl peroxide. An example of several combination preparations in the form of gels:

  • Klenzit-S – contains clindamycin 1% + adapalene 0.1%,
  • Indoxyl – contains clindamycin 1% + benzoyl peroxide 5%.

Important: for papulopustular acne of mild or moderate severity, antibacterial gels are prescribed primarily only in cases where the predominant inflammatory elements in the patient are pustules with pus. In this case, in addition to the external antibiotic, we must simultaneously use + external retinoids, + benzoyl peroxide.

And here you may ask: why is it recommended to also use benzoyl peroxide when prescribing an antibiotic gel? Isn't one antibiotic enough? The fact is that the use of benzoyl peroxide together with an antibiotic greatly reduces the risk of P.acnes bacteria developing resistance to antibiotics. Including the development of such resistance contributes to the transformation of mild forms of acne into forms with a more severe course.

As for patients whose inflammatory elements are represented predominantly by papules (i.e., without pus), it is irrational to prescribe antibiotics in this case. It is optimal to use only a combination of external preparations “benzoyl peroxide + retinoid”.

b) Papulopustular form of severe acne:

For severe papulopustular acne, antibiotics are prescribed only orally (i.e. in tablets for oral administration), plus we prescribe externally retinoids + benzoyl peroxide. For systemic antibiotic therapy, drugs from the tetracycline group will be primarily used - this can be either tetracycline itself or its derivatives - doxycycline and minocycline. Only a doctor can determine the optimal dosage for each individual patient.

As for tetracycline, the average daily dosage is usually 1000 mg, but in serious cases the dose can be increased to 3500 mg per day (such doses can only be used under monitoring of liver function). Tetracycline can be taken only 1 hour before meals or 2 hours after; the duration of administration is determined by the doctor, but not more than 8 weeks. However, this drug has serious age restrictions, and in recent years more resistant strains of P. acnes have been discovered.

Much fewer resistant P. acnes strains have been found to lipophilic tetracyclines (such as doxycycline and minocycline). Doxycycline is taken at a dose of 100 to 200 mg per day orally for no more than 8 weeks, regardless of meals. However, it induces photosensitivity in the summer and therefore in summer it is better to use another antibiotic (azithromycin). Minocycline is also taken at a dose of 100-200 mg per day, but this drug can cause blue-black pigmentation in different areas of the skin. This pigmentation most often occurs in the area of ​​atrophic scars and acne scars.

Antibiotics of the Macrolide group - previously a drug such as erythromycin was widely used, but now there are a lot of resistant strains of P.acnes to it. It is more preferable to prescribe azithromycin in a dose of 250 to 500 mg (taken only 3 times a week). Unfortunately, this drug may have side effects such as diarrhea, and therefore it is best to take it only with parallel medications to restore intestinal microflora.
